HealthCheck Screenings
Every Medicaid or BadgerCare eligible child up to age 21 is entitled to free, comprehensive health screenings. Wisconsin’s current schedule allows for a total of 29 screenings between birth and adulthood:
• Birth to 1st birthday – 6 screenings
• 1st to 2nd birthday – 3 screenings
• 2nd to 3rd birthday – 2 screenings
• 3rd to 21st birthday – 1 screening per year
SERVICES COVERED
A comprehensive health and developmental history review
A comprehensive unclothed physical exam.
An age-appropriate vision screen
A speech, hearing, and language development assessment and an age-appropriate hearing test
An oral assessment plus referral to a dentist beginning at age 3
Appropriate immunizations, based on age and health history
Laboratory tests based on age
sCHEDULE AN aPPOINTMENT
Call 1-800-722-2295 to find a local HealthCheck provider. All BadgerCare Plus and Medicaid HMOs are required to provide HealthCheck benefits.
After your appointment, request a written document of the date of the screening. This document needs to accompany future prior authorization requests for HealthCheck services. You can also use this standardized form.
Keep this document with you and show it at all referral visits or other periodic screening visits.
