Dr Thys Pienaar has just graduated as a Master of Science in Lasers in Dentistry with Honours. Dr Thys has stood out as a star student in the RWTH Aachen University's international class, flying the South African flag high, and continuing to ''wow'' daily in his surgery, not in the traditional way.

The 2-year post-graduate extra-occupational Master course consists of sound theoretical lectures and intensive skills training led by Prof. Dr. Norbert Gutknecht, university scientists, physicists and associates with doctorates. The Master course concludes with each participant completing, presenting and defending a Master thesis and ten clinical cases using various leading wavelengths.

Some Case Examples....

This case is an example where laser was used most of the time with minimal biological tissue removal, improving the success rate of the restorative dentistry with all the advantages of laser energy.

Diagnosis: Previous old root-canal filled tooth with a chronic apical infection resulting in a cyst-like formation around the buccal roots of tooth 16. Tooth 16 was fractured at bone level with the loss of the previous filling

Treatment Plan: As the previous root canal was filled with thermafil and both the MB1 and MB2 canals were filled with no detection of other canals, only an apisectomy was decided on the buccal roots with excavation of the cyst area. The total surgical treatment was performed with the Er,Cr:YSGG laser and a surgical curette. Due to the large area cyst opening bone regeneration (Sticky Bone) with the placements of PRF membranes for closure of the osteotomy window. The sticky bone and membranes were stimulated to initiate Photobiomodulation (PBM) with a specific laser wavelength before closure of the area. First a core build up was planned for primary healing followed by a longterm temporary crown.

Diagnosis: Old compromised root canal with a peri-apical area and decay under the crown

Treatment Plan: Removal old the old root-canal then inspect the root and do a small laser apisectomy with a new temporary crown
