2016 and Beyond with Sample Supports Sharing all we've done this year and where we're headed in 2017!

We are so excited to ring in the Holiday Season and New Year with each of you and wanted to share updates regarding our program services, our agency, and what you can expect from us in 2017! We are looking forward to continuing to work alongside the communities in Boulder County, the Denver area, and Northern Colorado to provide wrap-around services for the individuals we serve.

Supported Community Connections

Northern Colorado and Denver are proud to announce their new locations! The NoCo team is now located in Downtown Fort Collins and our Denver team is growing in Olde Town Arvada. We are excited to become a part of these communities and share our mission!

Each of our Day Programs are participating in volunteer opportunities to increase community integrated activities for the individuals we serve! We are supporting the individuals we serve to be present in their communities while building relationships with local businesses and the surrounding community members. It has been such a rewarding experience to see communities embrace our teams and we are looking forward to expanding our volunteer sites in 2017 to get even more involved in the communities we love!

Our Longmont Day Program team has been an integral part of the Longmont Downtown Development Authority’s event promotion. Individuals have the opportunity to visit local residences and businesses to share exciting events, like Small Business Saturday and the Holiday Parades, while introducing themselves as part of the Longmont community.

Outside of volunteering, our groups across regions have been enjoying all that Colorado has to offer with outdoor adventures, local tours, and seasonal events. We work to support each of the individuals we serve in accessing everything we all love about Colorado to the fullest extent regardless of their support needs. We enter the community as a team and have been embraced with positivity all around town!

In addition, our Leadership teams are partnering up with the Longmont Police Department to do an educational and informational training about individuals with intellectual and developmental disabilities to increase positive interactions with law enforcement. Our goal is to support the Police Department in contacting an individual’s support team in the event of an interaction to support a decrease in unnecessary legal involvement. All too often, the individuals we serve struggle to communicate or advocate for themselves in instances of law enforcement involvement and we are looking forward to having the opportunity to increase awareness of their unique needs. This collaboration has the potential to decrease inappropriate charges and increase the opportunity for the individuals we serve to have access to additional needed services in their communities instead of in institutionalize settings. We are really looking forward to this and will update everyone as we continue to collaborate!

Supported Employment

We are always seeking new community partnerships to expand our competitive and integrated employment opportunities through our Supported Employment Program. In Boulder County, we have recently grown our contract with the Longmont Downtown Development Authority and participated in hanging the holiday banners around Main Street. It was a blast and you can see all of the team’s hard work as you drive by! Our team is excited to be a part of the holiday events happening local to Longmont and the individuals we serve are an integral part of the scene!

Our North region has been working on finding various community partnerships in Fort Collins. We've expanded our employment opportunities to include several businesses in Old Town Fort Collins, including The Whiskey and Tony's. Our goal is to keep the opportunities growing across all types of local businesses!

Down in Denver, our team is growing their contract with Gallery 1874 and our newest location of Crystal Joys! There are endless opportunities across Olde Town and team Denver is looking to find the perfect jobs for each of the individuals we serve.

Our partnerships with Samples World Bistro, Gallery 1874, and Crystal Joys have already been rocking the holiday season with even more positions and opportunities to get individuals employed with jobs they can be proud of. Each member of the Samples World Bistro, Gallery 1874, and Crystal Joy’s teams support the growth of the individuals we serve and make it super fun along the way! We love seeing everyone’s families and support teams in for the holiday events and hope to see you all at our Downtown locations!

Residential and Behavior Services

We are always looking for even more amazing Host Home Providers across regions! These opportunities are truly unique as we put high emphasis in our matching process to ensure long-lasting and successful placements. Interested?

Our Supported Living Services Programs are continuing to grow as many individuals were provided with funding for the first time this year! It has been a privilege to provide a variety of supports both in-home and in the community for individuals who were previously denied access to funding for services. Our SLS team works hard to advocate for best individual support plan for each of the individuals we serve and look forward to tracking progress along the way. We have been focused on developing strong and trusting relationships with local resources to support the individuals we serve in being successful and independent in their communities. Our SLS team is there to provide mentorship and advocacy services to ensure each individual in service’s progress is limitless!

Our teams also specialize in advocating for the individuals we serve to have access to increased support services as needed to be successful in community-based settings. We began Sample Supports with the mission of providing services in the least restrictive environment for individuals with intensive needs. In 2016 alone, we have successfully supported 6 individuals in transitioning out of institutionalized settings to community-based settings and they are thriving! We have even received national recognition for our work here and have increased our contacts outside of Colorado as a specialized provider for individuals who are struggling to be served effectively. There is endless work to be done in advocating for community-based supports and we look forward to another year of progress!

Our community-based residential opportunities are also continuing to grow in success for each home and independent apartment we serve! The Denver team has welcome their first supported apartment placement which is an exciting start to a growing program!

The Residential Programs are focused on creating long-lasting placements and supporting placements for individuals who have struggled to maintain an independent or shared home. With the holiday season underway, we are proud to have each of the individuals we serve in a fulfilling and supportive placement where they can look forward with positivity in the New Year! Partnered with our Behavior Services Programs, we are able to support individuals in gaining the skills and confidence to be successful in the community. We are continuing to expand to our specialized support of dual diagnoses for individuals with mental health and substance use needs as we see this population being drastically underserved. Instead of redirecting supports to agencies who are unfamiliar with the unique needs of individuals with intellectual and developmental disabilities, we embrace the challenge and support the individuals we serve in receiving individualized support plans to reach their goals.

Our newest adventure for 2017 will for the opening of Sample Supports Youth Services where we will provide Foster Care and additional comprehensive services for children with intellectual and developmental disabilities! We cannot wait to be able to support the community in finding secure and therapeutic placements for children seeking families. Stay tuned for more updates as we get started!

Please be on the look out for our holiday appreciation event as well! We will be hosting a festive get together for all of the individuals we serve to attend where we can celebrate and get excited for everything to come. We support each guest in making holiday crafts and gifts for their loved ones and everyone is able to celebrate together before the long holiday weekend.
