Values Into Action NJ- Supporting people with disabilities & their families since 2007. Values into Action NJ provides Support Coordination in service for people with disabilities aged 21+ and their families, across the state of New Jersey. Using a person-directed, family-centered approach, we structure our services to assist individuals through the process of self-direction and empower them to remain in charge of their plans (and their lives).

We provide support coordination which is case management services for people with intellectual and developmental disabilities who are eligible for DDD. Support Coordinators are responsible for utilizing a person-centered planning process to identify outcomes, planning goals, and supports needs and develop/maintain the Individualized Service Plan (ISP). In addition, the Support Coordinator also help the person and his/her family find supports that best meet his/her needs and preferences. The Support Coordinator also conducts ongoing monitoring of the services included in the ISP.

We work with individuals of all ages to provide an array of services that promote choice, personal development, and personal growth. Services range from Support Coordination to Community Based Supports through the NJ Division of Developmental Disabilities. We also provide counseling and consultation through our private practice which supports individuals/families faced with various emotional difficulties and mental health issues. Our services are tailored to meet the individual needs of each person served.

PRISM Support Services is a DDD Approved Support Coordination Agency serving all Counties in New Jersey. Are you turning 21 and getting ready to graduate high school? Equal Opportunity Support Services helps youth transition into DDD adult services. Equal Opportunity Support Services will attend IEP meetings to support the family and the Child Study team; and answer any questions about the DDD application process.

Mercy Drive Regional Corp. (MDRC) is a human service agency. We provide services to individuals diagnosed with developmental and intellectual disabilities. MDRC is a multi-cultural agency that provides services in several languages taking into consideration the cultural diversity of New Jersey. MDRC recognizes the need for family involvement in the lives of individuals with developmental disabilities, by participating in the planning of services, irrespective of language barriers, thereby honoring/preserving/promoting the individual’s cultural background and diversity. We provide Service Coordination service to assist individuals with developmental and intellectual disabilities in gaining access to needed program and state plan services, as well as needed medical, social, educational and other services.
