Out of the 30 members on the 2017-2018 Cherry Health AmeriCorps team, 23% became employed with Cherry Health, 20% went to graduate or medical school, and 27% signed on for an additional year of service with Cherry Health AmeriCorps

91% of eligible patients received depression screening and follow-up organization wide
83% of adults and 75% of children were assessed and provided BMI/nutrition/physical activity counseling
72% of diabetic patients have diabetes under control

Upgrade to Heart of the City X-ray technology and services, producing higher quality imaging and faster Radiologist interpretation

Eaton Community Health Center screened 70% of patients for colorectal cancer
Heart of the City Health Center Adult Medicine reached 115 seniors through Senior Program
At Cedar Springs School Health Center, 95% of patients are current with BMI screening and follow-up and 97% are current with depression screening and follow-up
Provided diabetic eye care to a record high 2,048 patients across all three vision locations

Addressing the National Opioid Epidemic

Opioid alternatives and tapering programs are available to patients

Increased number of patients treated for substance use disorder by 53% by providing medication assisted treatment in primary care settings

Decreased opioid tablets dispensed by the pharmacy by 71% over four years
Treated over 50 Durham Clinic patients using Vivitrol/Suboxone combined with counseling

Southside Health Center recovery services staff helped create a Community Based Narcotics Anonymous meeting specifically for medication assisted treatment patients

Launched pilot program to offer virtual substance use disorder services in rural areas (tele-SUD)

Created partnership with the Red Project to enhance harm reduction efforts for patients in the medication assisted treatment program

Savings for Patients

Covered $1.2 million patient lab costs through partnership with Quest Diagnostics

Provided assistance to pay for mammograms for more than 300 women in several communities

Provided education to 5,092 individuals about the importance of mammograms and women’s health

The Pharmacy funded a Hepatitis C program for patients through 340B savings. The program has a 96% completion rate.

Received 2018 HRSA Health Center Quality Leader and 2018 Advancing Health Information Technology (HIT) for Quality Awards
Nine primary care medical homes received quality awards from Priority Health
Achieved Blue Cross Blue Shield Patient Centered Medical Home Designation at Barry Community Health Center, Cherry Street Health Center, Heart of the City Health Center Adult Medicine, Heart of the City Health Center Durham Clinic, Heart of the City Health Center Pediatrics, Montcalm Area Health Center and Westside Health Center
Received full accreditation by AAAHC
The Vision Department received accreditation for the new Community Health & Primary Care Optometry Residency program (the first of its kind in the Midwest) by the Accreditation Council on Optometric Education (ACOE), which is the only accrediting body for professional optometric degree (O.D.) programs and optometric residency programs in the United States and Canada
The Research department had three high impact studies accepted for national publication in ‘BMC Psychiatry,’ ‘Schizophrenia Research,’ and the ‘Journal of Clinical Psychiatry’

Vaccinations

Burton Health Center’s adult vaccine rate increased from 14% to 79.7% and the tetanus vaccinate rate increased by 96%

Cherry Street Health Center increased childhood immunizations to 71%

72% of patients were up-to-date on their HPV vaccination (compared to 41% statewide) and 89% were up-to-date on meningococcal vaccine (compared to 45% statewide) at Cedar Springs School Health Center

98% of school based health center patients were up to date on immunizations

Partnered with Molina Healthcare to sponsor first ever Flu Shot Campaign contest; Cherry Health Pediatrics declared victory

National Health Center Week

Held a superhero staff photo contest
Partnered with Blue Care Network to provide costumes, prizes and more for pediatric patients
Hosted five pop up events in collaboration with the Urban Core Collective
Partnered with Spoke Folks to provide bikes and helmets to two pediatric patients

School Programs

School linked traveling dental program placed over 10,000 sealants for students

Patient and Community Education

Provided prevention education to 4,884 individuals through Health Promotion department
Started Walk with a Doc program at Cherry Street Health Center

Increased internship opportunities to include health information management, billing, public health administration and corrections

Assisted 13,578 individuals in connecting them with health insurance
Nearly 70 providers participating in National Health Service Corps (NHSC)

Breaking Down Health Disparities

The vision department improved access to diabetic eye care services by 13% for LatinX patients within Burton Health Center
Implemented Social Determinants of Health screenings in the primary care setting and screened 3,231 patients
Provided training for 106 bi-lingual staff to become medical interpreters through national Bridging the Gap program

95% of youth completing the Yo Puedo program increased their knowledge of alcohol, tobacco and other drugs

AmeriCorps members provided interpretation for 19,839 patients, for a total of 2,933 hours of interpretation services

Implemented prescription refill reminder calls in English and Spanish in an effort to increase adherence

Corrections

Achieved 88% resident completion rate
82% of all residents maintained their jobs longer than 30 days

Clubhouse Programs

Opened Lakeside Clubhouse in Holland
Sheldon House received 2018 Partners in Excellence Award from the Community Mental Health Association of Michigan
Sheldon House received a full three-year accreditation from Clubhouse International

Advocacy

Achieved Silver Advocacy Center of Excellence through National Association of Community Health Centers (NACHC)
Hosted seven elected official visits
Sent first ever Cherry Health staff member through the Advocacy Leadership Program through NACHC
Over 60 staff participated in first ever staff voting contest
100% board and staff members signed up as health center advocates

Maternal Infant Health Program (MIHP)

Received $90,000 grant from Blue Cross Blue Shield of Michigan to provide healthcare services for high-risk expectant mothers and infants

Through the Michigan Health Endowment Fund, added two Infant Mental Health Specialists to improve the mental health of new mothers under our care

Cherry Health Foundation

817 households and organizations donated to Cherry Health Foundation – 38% more than last year

90 employees donated to Cherry Health Foundation for the first time

38% more donors gave to Cherry Health Foundation, and annual fund gifts increased by 18.2%

Over 200 community members rallied around our first luncheon event, introducing new friends to Cherry Health’s mission

More guests and sponsors supported our Cherry Health Celebration fundraising event than ever before
