Along with registering, you must sign and present the last 3 pages in the Handbook (Handbook agreement form, Conditions of Enrollment, COVID waiver form) on at the mandatory parent meeting.

Note: The following documents must be filled out and turned into the parish office, if you are not an Assumption parishioner, with a copy of your child(ren)'s Baptismal certificate (if you are new to the program) by Sept. 18th.
